Home Health Services
Our home health services are administered in the patient’s home by professionally trained and highly experienced home health care staff. These services are covered by Medicare, Medicare Advantage Plans, and certain commercial insurance plans. Your recovery will be achieved by intermittent home visits, the frequency of which will be determined by your physician and our assessing clinicians. Home health services are ideal for patients who require Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Services, and/or Certified Home Health Aides.

Who should I call for Home Health Care Services?

Residents of Collin, Dallas, Denton, Ellis, Fannin, Grayson, Hunt, Kaufman, and Rockwall counties should contact the Rockwall or Sherman Offices for home health services.
​Residents of Tarrant County, Northern Johnson County, and Irving should contact the Bedford Office for home health services.
Who is eligible for Home Health Care Services?​
​
Eligible individuals are homebound and require the need for skilled nursing care and/or therapy services.
​
Homebound is defined by Medicare as “considerable and taxing effort to leave the home, infrequently, and for short durations of time.”
​
A physician must order home health services. We can assist you in obtaining an order from your current physician.
