Who pays for home health care services?
Your cost will be covered 100% if you are a Traditional Medicare patient. Some Medicare replacement and commercial plans have copays, and your coverage may vary depending on your plan. We would be happy to run your insurance and discuss coverage with you.

Who would benefit from home health care services?
Home health care can assist you if:

You are recovering from hospitalizations, surgeries or illnesses

You have recently been discharged from rehab

You have a chronic condition such as COPD, CHF, diabetes, etc.

You have a chronic or acute wound and need wound care

You need education on your medications or disease process

You have frequent visits to the doctor’s office or a hospital

You have frequent falls
-
You have a sudden onset of cognitive issues

You have a decline in mobility or normal function

You have had a sudden loss of appetite or difficulty swallowing

You have pain that is difficult to control
How do I know if I’m eligible for home health care?
All of the following must apply:

You must be homebound, meaning your condition limits your ability to leave home without assistance.

Your doctor must write an order for home health services. We can assist you with obtaining the order if needed.

You require either skilled nursing care or therapy services on an intermittent basis.
Can I request a specific home health agency if my doctor has ordered home health services?
Yes! It is your right to choose the home health agency that you prefer. You should never let anyone pressure you into using a specific company.
What can I expect from my first home health care visit?
At Sunrise, each patient will receive a customized care plan based on an initial assessment and discussion with your referring doctor. This initial visit is performed by either an RN or a therapist. Your care plan is completed after the clinician performs the first visit, during which a head-to-toe assessment will be conducted along with a patient/caregiver interview to gather medical history and determine your needs. You will benefit from our team approach, as we will work with you, your family, your physician and other medical providers to provide you with the highest level of care!

When will my visits start?
Your initial home health visit is typically performed within 24-48 hours after we have received a referral from your physician or after you have discharged from a hospital or rehab. You will be contacted by your assessing clinician to schedule a time for the initial visit that is convenient for you.

How often will I receive visits from Sunrise’s home health clinicians?
This depends on your condition(s). The frequency of visits is determined by your physician and your home health assessing clinician. The frequency may vary according to your doctor’s orders and any change in your condition(s) while on service with home health. Your insurance coverage may also affect the number of visits that you receive.
How do I get started on home health services? Call your doctor and request an order for home health care. Remember, it’s your right to choose the home health company that you want to use. We will assist you with obtaining an order from your physician for home health care if necessary. Please contact one of our branch offices if you have further questions or needs. PERSONAL ATTENDANT SERVICES FAQs

Who pays for PAS?
You must be approved for Medicaid to qualify for free services.

How do I apply for PAS?
Please contact the Department of Aging and Disability Services at 1-888-337-6377 to apply for Medicaid and Personal Assistance Services. You can also contact 211Texas.org or call 211 and ask for the nearest provider.

Are there any additional qualifications for PAS?
Yes. You must be a Texas resident and have an income that is below the federal poverty level. You must also have a qualifying disability, and affirmation from a doctor that PAS services are required.

How do I transfer to your PAS agency from another agency?
If you have Medicaid, please call your caseworker from the Department of Aging and Disability. If you have an MCO, call your insurance provider and notify them that you want to change agencies.

Can a family member or a friend be my Personal Attendant?
Yes – with the exception of spouses. The attendant must be at least 18 years of age and must pass a background check.
